During the appointment, our team will perform critical clinical screenings, including checking your blood pressure and cholesterol (cardiovascular health), reviewing your recent HbA1c blood tests, checking your kidney filtration function, and confirming your retinal eye screening referral status. Please bring your home blood glucose monitoring device (meter or logbook or CGM readings), a current list of all your medications (including any over-the-counter supplements), and wear comfortable footwear, as our nursing team will need to perform a physical foot and circulation check during this appointment.

Persistent high blood sugar can damage the delicate filters inside your kidneys over time. By running a routine blood and urine test during your free annual review, we can catch the earliest signs of diabetic kidney strain and adjust your treatment plan to protect your long-term health.
